Description: The DSRIP Behavioral Health Assessment Specialist functions as a member for the treatment team and is responsible for providing mental health services. This job also provides direct care and other support services in a way that promotes positive treatment outcomes and meet the needs of our behavioral health patients. Typical Duties:
Diagnoses and provides recommendations in levels of care for the treatment of mental disorders and/or substance abuse disorder.
Provides quick and accurate clinical assessments of mental health and substance abuse conditions.
Conducts intake assessments, triage, referrals, case management and crisis intervention.
Documents treatment goals and plans in the electronic medical record of which includes all assessments and all relevant clinical documentation.
Assists the treatment team in recognizing, treating and managing mental health and psychosocial issues.
Utilizes behavioral health expertise to promote change and improve behavioral and physical health conditions.
Provides care-management skills and knowledge of local resources for outside referrals.
Advises the care team about which clients are better served at in a different setting as well as and which should be referred to other mental health facilities or specialties.
Develops relapse prevention plans and helps clients maintain stable functioning, when necessary.
Monitors and coordinates the delivery of health services for clients related to behavioral health , including linking with other treatment providers not only within the primary care setting but, outside it as well.
Assists in process for any recommendations that need required authorizations prior to arranging for treatment.
Provides clients with self-management skills and educational information, so that, they can be a full participant in their own treatment and recovery.
Performs other related job duties as assigned.
Required Education and Experience:
Master’s Degree in Counseling or Social Work.
2 plus years of work experience conducting diagnosis of severe mental illness and/or substance abuse disorders in a healthcare, therapeutic, physician, and/or other clinical practice setting.
Must have a current licensure in one of the following: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners or the Texas State Board of Examiners of Professional Counselors.
Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment.
